Group and Centre Based Activities
From relaxing craft activities to exploring the world of technology, the Connecting Communities Centre will provide an exciting a friendly atmosphere. All activities are designed for clients to learn new skills, enjoy their time, socialise and make friends and to feel safe. All groups vary from a 1:1 basis to a maximum number of 1 support worker to 5 participants. We want your time at the Connecting Communities Centre to be fun! Please let us know if there's something you want to do or learn and we will do all we can to assist you.
NDIS Funded Community Access Supports
Under the NDIS, Connecting 2 Community Circles (C2CC) can provide supports to connect Participants with the community. Our primary goal is to provide the necessary supports to enable people to become a confident and contributing member of the community. The activities of Community Access are driven by the goals and needs of the individual.
With a strong participant focus, C2CC can help you with a diverse array of activities outside your home, which may include:
Our dedicated team will work with you to create a daily routine, plus support you in daily activities to connect with the community. We provide trained support workers who are able to support each person to achieve their individual goals.

- Social occasions.
- Recreation activities and events
- Sporting events
- Developing personal interests or hobbies.
- Shopping and attending appointments.
- Exercise and mobility
Our dedicated team will work with you to create a daily routine, plus support you in daily activities to connect with the community. We provide trained support workers who are able to support each person to achieve their individual goals.
Assisted Daily Living
Having support with everyday tasks such as getting ready for the day, or cleaning, allows you to live independently at home, with the assistance from our experienced staff. Our dedicated Support Workers can assist you with your daily personal care and activities including:
- Personal Hygiene and grooming.
-Toileting
-Eating and Drinking (meal times)
- Attending appointments
-Mobility and Transfers
-Meal preparation
Our services aim to maximise your independence and functional skills. Personal Care Supports can be delivered across a variety of settings, including in your own home, and when undertaking social, recreational, education activities away from the home.

PRICE GUIDE
Connecting 2 Community Circles prices align with the National NDIS Price Guide. We utilise the Temporary Transformation Payment (TTP)
