Our organisation has a strong focus on helping all members of the community, and focusing on those individuals who need additional support in areas such as maintaining independence at home and in the community.

what we can offer
Community Access
Connecting Communities provides supports assisting with Community Access. Activities can range from visiting the library, joining a social club, or attending appointments and social outings. |
Assisted Daily Living
We also offer support to NDIS participants with in-home supports. These can include assistance with personal hygiene, eating, mobility and transfer, and general cleaning duties. |
Nursing
Our nursing scope is governed by a Registered Nurse with over 30 years experience in acute and community nursing. We can provide nursing assessments & clinical care. |
Accommodation
Connecting Communities is proud to provide Supported Independent Living, with one self contained property. This is available for short-term, and respite services. |
