Connecting Communities is a registered NDIS provider, delivering supports since 2017 to the Townsville community and surrounding areas. Our focus is on those individuals with a disability to provide meaningful supports in assisting clients to achieve their goals.

With such goals in mind it is essential to implement and maintain a person centered approach, of which is done frequently at Connecting Communities. Proactively providing extensive training and ongoing education to the support team to build and maintain a high level of skill. We aim to provide competency based training, delivering education to support workers, family members, and individuals. Connecting Communities also engages in external training for the surrounding areas of Townsville, from Cairns to Mackay.
At Connecting Communities, our organisation has a strong focus on helping all members of the community, and focusing on those individuals who need additional support in areas. We provide a range of NDIS services including community access, assistance with daily living, supported independent living, respite, short-term and medium-term accommodation, and community nursing.
We currently have one Registered Nurse, and three Endorsed Enrolled Nurses carrying out the community nursing program. Through this program, we can provide assessment, education, and health monitoring to all participants. We provide education to individuals, heath workers, and external organisations requiring support and education to support their clients.
Connecting Communities is also nursing governed, which ensures that we have a work force that can assist and provide supports to high complex clients requiring nursing assessment and monitoring, with evaluation and reassessment. We have a strong management portfolio of highly skilled and experienced Team Leaders providing ongoing education mentoring and support to the care team. In doing so we are ensuring best practice and quality assurance of all staff members, a critical part of our business operations.

At Connecting Communities we also have a wheelchair accessible bus which has enabled the organisation to assist individuals requiring wheelchair accessible transport access to our services in the one stop shop.
Through our journey as Connecting Communities we have always believed that it’s our clients and people in the community that are our priority. We will continually assess the services we provide by giving all our clients and families visible access to provide ongoing feedback to our organisation and to monitor the service they are being given through our cloud-based system, Flowpoint. Flowpoint gives our clients and families online access to their own roster of care, progress notes, support team, compliments and complaints, their service agreements and NDIS paperwork. It is important that all our clients and families are part of the support team.
