Connecting Communities offers support to NDIS participants to access social, community, recreational and volunteer opportunities in the community. We can provide support to community activities and events such as:
  • Visiting the library
  • Going to the cinema
  • Joining a social club/group
  • Attending sporting events
  • Personal shopping
  • Attending appointments
  • Social outings​​

​We have a Principle Registered Nurse, and we also have three endorsed enrolled nurses working within our team. ​​Our nursing team are able to provide an array of assessments and management plans for various health related assessments including:

​Connecting Communities is proud to provide Supported Independent Living, with two self contained properties. These are available for SDA, short-term, and respite services boasting the following:
